Administering IV Captopril: Dosage and Protocol

IV captopril is administered slowly, typically over 5 minutes. The usual starting dose is 6.25 mg to 25 mg. Always dilute the medication before administration. A 25-mg vial of captopril is commonly diluted in 50 ml of compatible intravenous solution such as D5W or normal saline.

Closely monitor the patient’s blood pressure during and after administration. Hypotension is a common side effect, particularly in patients with volume depletion or pre-existing hypertension. Have emergency medications readily available to manage hypotension.

Adjust the dosage based on the patient’s response and tolerance. Subsequent doses, if needed, can be administered at intervals determined by the treating physician, but typically not within less than 6 hours. Patients should be monitored for any allergic reactions, which can manifest as rash, itching or difficulty breathing. Discontinue administration immediately and treat appropriately if an allergic reaction occurs.

Renal function should be assessed before and during treatment as captopril can impair kidney function, particularly in patients with pre-existing renal disease. Regular blood tests to monitor creatinine levels are recommended.
